The temporal bone is one of the most complex structures in the human skull — and one of the most important for hearing. Located on either side of the skull, this paired bone houses the delicate structures of the middle and inner ear, making it central to how we perceive sound. Understanding the temporal bone and its anatomy can help shed light on hearing health, balance and why certain conditions or injuries in this region can have such a significant impact on everyday life.

Temporal bone anatomy

The temporal bone sits at the base and sides of the skull, forming part of the cranial floor and the lateral wall of the cranium. It articulates with several other bones, including the parietal, occipital, sphenoid and zygomatic bones and plays a key structural role in protecting the brain while housing critical sensory organs.

Temporal bone anatomy is notably intricate — it contains the cochlea, semicircular canals, ossicles and the facial nerve canal, among other structures. Because of this complexity, the temporal skull bone is studied in detail in both audiology and otolaryngology (ear, nose and throat medicine).

The bone is typically divided into several parts, each with distinct functions: the squamous part, the petrous part, the tympanic part and the mastoid part. Together, they form a unified structure that serves both protective and sensory purposes.

The mastoid portion of the temporal bone

The mastoid portion of the temporal bone is the rear section of the bone, recognisable as the bony prominence you can feel just behind the ear. It contains air-filled spaces called mastoid air cells, which are connected to the middle ear and help regulate air pressure within the ear.

The mastoid process of the temporal bone — a downward-pointing projection — serves as an attachment point for several muscles of the neck, including the sternocleidomastoid. While it may seem like a simple bony bump, the mastoid process plays an important functional role and is a key landmark in temporal bone anatomy.

The mastoid region is also clinically significant: infections of the middle ear can spread to the mastoid air cells, leading to a condition called mastoiditis, which requires prompt medical attention.

The squamous part of the temporal bone

The squamous part of the temporal bone — also referred to as the squamous portion or squamous portion temporal bone — is the large, flat, fan-shaped section that forms part of the side wall of the skull. It is the most superior portion of the bone and contributes to the temporal fossa, which is the shallow depression on the side of the skull.

The squamous of temporal bone is notably thin and smooth on its outer surface, where the temporalis muscle — used in chewing — attaches. On its inner surface, it features grooves for meningeal blood vessels.

The external auditory meatus of the temporal bone

The external acoustic meatus of the temporal bone — more commonly called the external auditory meatus — is the canal that runs from the outer ear (the pinna) inward to the eardrum (tympanic membrane). It is partly formed by the tympanic part of the temporal bone and partly by cartilage in its outer section.

The external auditory meatus of the temporal bone channels sound waves from the environment toward the middle ear, making it the first bony structure involved in the hearing process. Its shape and dimensions influence how effectively sound reaches the eardrum and any blockage or abnormality in this canal can affect hearing.

The temporal bone anatomy includes several important projections and landmarks that are relevant both structurally and clinically.

Zygomatic process of the temporal bone

The zygomatic process of the temporal bone is a long, slender projection that extends forward and articulates with the zygomatic bone (the cheekbone) to form the zygomatic arch — the bony bridge you can feel running from your ear toward your cheek. The temporal process of zygomatic bone meets this projection to complete the arch, which serves as an attachment point for the masseter muscle, involved in chewing.

Styloid process of the temporal bone

The styloid process of the temporal bone is a slender, downward-pointing spike of bone located just below the external auditory meatus. It provides attachment points for several muscles and ligaments associated with the tongue, pharynx and hyoid bone. Despite its small size, the styloid process of the temporal bone is a notable anatomical landmark and an unusually elongated styloid process can occasionally cause discomfort or difficulty swallowing — a condition known as Eagle syndrome.

Temporal bone squamous part

The temporal bone squamous part also contributes the squamotympanic fissure and the articular surface for the temporomandibular joint (TMJ), the joint that allows the jaw to open and close. This connection means that temporal bone anatomy is relevant not only to hearing but also to jaw function and facial pain conditions.

The petrous part of the temporal bone

The petrous part of the temporal bone — sometimes called the petrous portion or simply the petrous of temporal bone — is arguably the most critical section from a hearing and balance perspective. The name comes from the Latin petra, meaning stone, reflecting how dense and hard this portion of the bone is.

The petrous part of temporal bone lies at the base of the skull, angled inward toward the centre of the cranium. Inside it are housed the cochlea (responsible for hearing), the semicircular canals (responsible for balance) and the internal auditory canal, through which the vestibulocochlear nerve (cranial nerve VIII) and the facial nerve (cranial nerve VII) travel.

Because the petrous portion contains such vital structures, any damage to this region — from trauma, infection or tumour — can have serious consequences for hearing, balance and facial movement.

Temporal bone injuries: broken and fractured temporal bone

Given its location at the base and side of the skull, the temporal bone is vulnerable to injury in cases of significant head trauma — most commonly from falls, road accidents or sports injuries. A broken temporal bone or temporal bone fracture is a serious medical event that requires immediate evaluation.

What happens when the temporal bone is broken?

A broken temporal bone most often results from blunt force to the side of the head. Symptoms can vary depending on which part of the bone is affected, but may include:

  • Hearing loss (conductive or sensorineural)
  • Tinnitus (ringing in the ears)
  • Dizziness or vertigo
  • Facial nerve paralysis or weakness
  • Bleeding from the ear canal
  • Bruising behind the ear (known as Battle's sign)

In some cases, a break may damage the structures of the inner ear directly, leading to permanent hearing changes. In others, the injury affects the ossicles or eardrum, which may be treatable.

Fracture of the temporal bone

A fracture of the temporal bone is classified based on its orientation relative to the long axis of the petrous bone. Longitudinal fractures — running parallel to the petrous ridge — are more common and typically affect the middle ear. Transverse fractures — running perpendicular to the ridge — are less frequent but more likely to damage the inner ear and facial nerve, often resulting in more severe hearing loss.

Mixed or complex fractures can involve elements of both. Diagnosis typically requires a CT scan and management depends on the structures involved. Hearing loss following a temporal bone fracture should always be assessed by a hearing care specialist, as early intervention can make a significant difference to outcomes.
