These are Miracle Ear d.b.a. Amplifon Canada Inc.’s (hereinafter referred to as “Amplifon Canada”) Rewards Programs (“ Rewards Programs”):

The person being referred must be a new customer to Amplifon.
For both parties to be eligible for the reward, the referred friend or family member must provide Amplifon with the following information at the time of booking their hearing assessment: The full name of the person who referred them, the referrer’s phone number, and the Amplifon clinic the referrer visits.
To be eligible for the $10 Tim Hortons® gift card, the referred individual must complete a hearing test appointment.
The referrer may inquire at the clinic 1–2 weeks after the referred person’s completed hearing test to receive their Tim Hortons gift card.
When the referred person purchases a hearing solution from Amplifon with a minimum value of $1,000, both the referrer and the referred person will receive a $100 Shoppers Drug Mart® gift card.
$100 Shoppers Drug Mart gift cards will be available for pickup at the clinic. Please allow 4–6 weeks after the qualifying event for them to be ready. The clinic will contact both the referrer and the referred person to arrange pickup.
The referred person becomes eligible for their reward after completing the trial period for their hearing aids.
